Breast Cancer Support Project
Assists newly diagnosed or patients just released from the medical system with an emergency fund set up for rent/mortgage or utility assistance. Anticipates assisting a minimum of five women. Breast Cancer Support Project
Provides support services to at least 60 women a year, with an average of five new women accessing services every month. These services include: transportation to medical appointments, child-care, housekeeping, food delivery and emotional support for homebound patients. All the women served are low-income, living at or below 200 percent of the federal poverty level. Recipients are residents of the greater Sacramento area, which includes Placer, Sacramento and Yolo counties.

Support Navigator/Advocate Project
This program recruits breast cancer survivors to work with newly diagnosed low-income women referred by early detection programs and helps them get established in care. The volunteers are trained by a team of local treatment providers, survivors and advocates and follow an established curriculum. Recipients are residents of the greater Sacramento area, which includes Placer, Sacramento and Yolo counties. Anticipates assisting a minimum of 30 women.

Hmong Community Wellness Project
Provides breast cancer education for the Hmong community in Butte County. Project staff will: 1) conduct presentations to the Hmong community to increase breast health knowledge and screening practices and decrease harmful farming practices; 2) conduct a needs assessment with Hmong elders and community leaders to determine their needs related to local cancer support services; and 3) create a breast cancer and pesticides curriculum for breast cancer advocates and community agencies to utilize within the Hmong community.

Be a Voice for Wellness
Provides innovative breast cancer education to Native Americans by sponsoring talking circles and educational interventions. More classes will be added in Butte County and classes with be expanded to Colusa County.

Care Coordination and Navigation Home Support Project
This program is offered to uninsured and underinsured, low-income breast cancer patients residing in the Komen Sacramento Affiliate service area. The program will be expanded to include financial assistance for home rental or mortgage payments and food. Up to $600 per participant (approx. 80) is available for housing and $150 per participant (approx. 60) is available for food. Housing payments will be mailed directly to landlords or mortgage companies, and food vouchers will be issued from grocery stores convenient for the recipient.

Breast Cancer Screening, Education, Outreach and Case Management
Provides screening to low-income, uninsured and underinsured women in Yolo County, many of whom are further challenged by language and cultural barriers and have had little or no access to preventive health care services. Educational information will be provided and includes bilingual information about breast examinations and mammography, locations of CommuniCare's four primary care clinics in Yolo County, referrals to mammography services, and assistance in coordinating transportation to clinics and appointments. Anticipates serving a minimum of 1,010 women.

Breast Cancer Education, Screening and Early Detection
Provides low-income, uninsured and at-risk women between the ages of 14 and 39 with breast health education, as well as early detection and diagnostic services for breast cancer. Further outreach will be conducted in order to increase breast health awareness and services in the Stockton and Lodi areas.

Provides diagnostic services to 150 low income-women who are less than 40 years of age residing in Butte, Sutter and Yuba counties.

Mujeres Informadas
This program will increase the number of women screened for breast cancer in low income and medically underserved areas of Stanislaus County. Women will be trained to become promotores (lay health workers). Promotores provide health assessments, breast health education, resources, support and referrals to women in their communities through door-to-door contacts, community events and community education sessions.

PET - Positron Emission Tomography
This program will expand clinical imaging to include a new modality called positron emission mammography (PEM), a small, high-resolution PET scanner dedicated to breast imaging which can detect 1.4 - 2.0 mm malignancies in the breast. PEM can help identify the most accurate site for biopsies and help surgeons with breast conservation planning.

Peer Support Services and Education
Provides breast cancer peer support and breast health education to patients in the Sacramento Valley. Anticipates serving 125 breast cancer patients. Will provide: 1) six bi-monthly support groups led by breast cancer survivors; 2) medical interpretive services; 3) transportation support to Spanish breast cancer patients; and 4) a Spanish peer support 24-hour breast cancer hotline. Save Ourselves will also distribute 1,500 pieces of Spanish breast health educational materials in outreach efforts.

Patient Navigation for Spanish Speaking Breast Cancer Patients
Addresses the needs of underserved women over 40 aged residing in San Joaquin County who lack health insurance and/or family support and speak only Spanish. The nurse case manager and outreach worker will assist these patients by enrolling them in services established to help them as they undergo treatment and recovery. Transportation, medical interpretation and translation, treatment education and psychosocial support will be provided. Twelve support group meetings will be conducted.

Addresses the needs of underserved women of color over 40 aged residing in San Joaquin County. These patients will be provided free breast cancer risk assessments and scheduled for annual mammograms. They will also be enrolled in services established to help them as they undergo treatment and recovery. Transportation, translation and psychosocial support will be provided.

Breast Cancer Outreach and Education Program for Southeast Asian/San Joaquin
Addresses the needs of underserved women over 40 aged residing in San Joaquin County who are low income, face language barriers and/or lack health insurance. Special emphasis will be placed on women whose primary language is of Southeast Asian origin. Bilingual outreach workers will contact potential patients to educate them about the importance of annual screening and to assist them with scheduling mammograms.

Breast Cancer Navigator Program
Serves as a resource for breast cancer patients treated at Sutter Cancer Center as they navigate their way through the process of breast cancer diagnosis and treatment. Enhancements include: 1) providing information regarding chemotherapy to women newly diagnosed with breast cancer and who speak Spanish, Russian and Chinese; 2) providing five underserved women who have developed lymphedema with financial assistance to purchase treatment related supplies; and 3) revising My Personal Journey, A Woman's Guide for Breast Cancer Services, which includes information on treatment options, community resources, financial issues and Cancer Center programs.

Early Breast Cancer Screening and Case Management Project
Provides clinic based education, risk assessment and clinical breast exams as well as no-cost mammograms, diagnostic testing, and case management services to low-income women (age 20-39) in Sacramento County who are at high risk for breast cancer. Onsite child care and transportation to medical appointments may also be provided. Anticipates serving 200 women.

Breast Cancer Patient Navigator Program
Provides education, guidance, support and care coordination beginning at the point of diagnosis and continuing into treatment. Patients are contacted by the navigator shortly after their diagnosis but before they see their oncologists at the UC Davis Cancer Center for the first time. Together they discuss breast cancer, how it is treated, making decisions, preparing for surgery and adjuvant therapies, and discuss psychological and emotional concerns. Bi-weekly breast cancer education and orientation group sessions are conducted. The program will be expanded to provide support and information to all newly diagnosed breast cancer patients and to develop an educational package.

Shifa Clinic Healthy Breast Program
Addresses the needs of Middle Eastern and East Indian Muslim community members who face financial, language and cultural barriers in obtaining traditional healthcare services. The program relies on volunteer community physicians and UC Davis medical students and undergraduates to provide patient care to thousands of low-income, uninsured patients. A Promotores Model of healthcare outreach is used, with program Promotores going out into communities to provide information, awareness and screening to 500 women within the patient population and assuring that 100 women receive mammograms.

VN Cares Clinic Healthy Breast Program
Founded in 2001 the Vietnamese Cancer Awareness Research & Education Society
(VN CARES) addresses the findings that Vietnamese American women have the highest incident rate of cervical cancer - five times greater than Caucasian women. VN Cares implemented four goals to alleviate this alarming rate: (1) Inform the community about prevalent types of cancer, namely breast and cervical cancer, the benefits of regular cancer screenings and the available cancer treatments in a culturally sensitive and linguistically appropriate manner. (2) Enroll and schedule patients for free clinical breast exams, mammography, pap smears and the necessary follow-ups and treatments. (3) Remind and schedule recruited patients for annual cancer screenings (4) Provide patients with translational assistance and emotional support at their screenings. A Promotores Model of healthcare outreach is used, with program Promotores going out into communities to provide information, awareness and screening to 500 primarily Asian American/Pacific Islander women and assuring that 200 women receive clinical breast exams and mammograms.

Imani Clinic Healthy Breast Program
This program addresses the needs of uninsured, low-income African-American women living in Sacramento's Oak Park community. Open on Saturdays, the clinic provides culturally sensitive and linguistically appropriate care and relies solely on volunteer community physicians, medical students, undergraduates and community members. Using a Promotores Model of healthcare outreach, this program will provide healthy breast education, awareness and screening to 1,500 primarily African-American women and assure that 200 receive mammograms.

Healthy Breast Program
This program is a collaboration of five established clinics serving low-income neighborhoods in Sacramento, with its mission to provide free health services to uninsured and underserved patients who otherwise would not have access to care. The Consortium brings each of the clinics together to share best practices, collaborate on policies, procedures and common data collection and coordinate the solicitation of donations, pharmaceuticals and equipment. The Consortium will maintain the healthy breast patient survey, supervise Project Coordinators and Promotores for each clinic, maintain a common database, provide mammogram tracking/reminder reports, compile and compare data from each clinic regarding success rates for outreach, screening, mammograms, referral practices and outcomes and identify best practices.

ENCOREplus Breast Cancer Program
This program educates women of all ages about the importance of and necessity for early detection in treating breast cancer. Annual clinical breast exams, screening mammograms and breast ultrasound may be offered free for women who meet eligibility. Low-income and uninsured or under-insured women who may have symptoms of breast cancer and are 39 and younger are eligible. Because of their age, these women do not qualify for California State Programs. For women 40 and older, this program will qualify them for state-run programs and refer them to local providers for treatment. ENCOREplus offers support to women of all racial, ethnic and language groups in the Sacramento region and surrounding counties by maintaining contact with them and assisting them in getting free annual clinical breast exam, mammogram and/or breast ultrasound.
